Would you say smoking prevalence and deforestation are the biggest developmental issues in Indonesia? I'd think so, since they don't seem to have any rules restricting children smoking, with all the news about the 2-year old smoking in Indonesia. And how about deforestation? Is poverty the reason why people still rely on mass cutting down of trees? Keep in mind the keyword, "developmental" issues :) - It might be more relevant if people living in Indonesia respond, but you're welcome to share your thoughts.

the biggest issue is lack of educated workforce, or stable business-friendly government that can inspire confidence in foreign investors. Neither health and environmental problems will not get fixed until the country is developed. A hungry person does not care about environmental damage, and risk of lung cancer at 70 is not a big deal when you are expected to die from food poisoning at 50.
